Do fans have to be elevated off heatsink or better flush

But elevated usually doesn't survive jumps for long.
True, which is why I use aluminum fans. They hold up longer for me. In fact I’ve yet to break one.
 
I've experienced the wire to the fan board pulling away from the solder when elevated and bashing. Elevated or not, the plastic blades will get damaged from rocks if not protected with a mesh screen or those fancy carbon fan blade covers.
 
Elevated cools better with less of a dead spot. When the fan is placed directly against the heat sink, the center of the fan creates a dead spot where there are no blades. Also elevated allows debris to pass under without getting stuck as easy. Mine are elevated.
