Big Rock Anyone have issues when assembling slipper clutch?

I run into this problem when assembling the slipper clutch module every time. I assemble everything and screw it on, but it doesn't spin straight? It wobbles and basically feels like the screw isn't in alignment? I'm pretty sure I put the screw in perfectly straight but it always comes out a little wobbly

Sounds like the assembly is bent
 
One of the bearings in the module might be going out.
 
Literally a brand new assembly. I was putting a steel gear in it but I've come across this problem multiple times. It makes my car sound very bad.. like gears grinding lol in a bad way
Did you add the 4th slipper pad? And the steel spur makes a very diff noise. If you can upload a video to YouTube amd post it here. It'll help alot
