What happened here and how can i fix it?

The screw fell out inside of the shaft. It'll still be in there, just pull that half of the shaft and use a long Allen through the inside of the shaft to put it back on. Use some blue thread lock to keep it from coming loose again.

Here's a video on the other end of the shaft (diff end). It's the same thing on the axle stub end.

Mine did that. Easy fix once you figure out how to tighten and loosen the shafts by going through the inside like the video above shows.

I'm using stuff I've had for a long time. I think they are either Snap-On, Blue Point, Klein or Eklind T-handles. There's a complete section here on tools that would be a good place to get some ideas.
